Transition Curriculum
Curriculum Objective
The goal of this curriculum is to train physicians to be skilled and knowledgeable facilitators of the transition of youth with special health care needs from pediatric to adult oriented healthcare systems.
Curriculum Goals
- Understand the rational and indications for transition of youth with special health care needs.
- Understand the aspects of effective transition of youth with special health care needs in both the primary and subspecialty care settings.
- Develop the skills and knowledge to be effective physician facilitators of the transition process within a healthcare team.
- Understand common chronic diseases of childhood onset.
